  Introduction: Congenital cytomegalovirus (CMV) infection is one of the most common viral causes of congenital infections in developed countries.

  Case report: We report the case of a newborn infant with microcephaly, jaundice, hepatosplenomegaly and petechiae in whom the infection was confirmed by isolation of the virus from urine and the detection of CMV antigen in blood. The patient was treated with intravenous ganciclovir for 6 weeks. Magnetic resonance imaging (MRI) findings included multifocal lesions, predominantly involving deep parietal white matter and in anterior temporal lobe. We describe the MRI findings.

  Discussion: In patients with static encephalopathy, an MRI imaging pattern of multifocal lesions predominantly involving deep parietal white matter, with or without gyral abnormalities, is predictive of congenital CMV infection. The presence of abnormalities in anterior temporal lobe (with or without cysts) increases the likelihood of CMV infection being present. We have done a literature review, focusing especially on CMV neuroimaging-based studies.
