In the treatment of the child with attention deficit hyperactivity disorder (ADHD) we have to include the family from the beginning, mainly the parents, since it is going to be through them how our interventions can have a greater repercussion and benefit for the minor.
We propose as a pillar of any intervention, the psychoeducational groups of parents, in which they are given the information and the necessary tools to begin to understand the problem of the child, how to help him, the different alternatives of treatments and from there improve the alliance with the therapists who will intervene.
A psychoeducational program of 4 sessions of two hours each for parents with children with ADHD is established at the mental health center, directed by the psychologist and the psychiatrist of the children and youth program. At the end of each group an anonymous evaluation is done to have a feedback about how the parents had received the information transmitted with 4 questions, assessment of the sessions and the possibility of suggestions.
An analysis of the questionnaires collected during a school year is made, obtaining a very positive assessment in all the suggested aspects and suggesting to expand them or include more topics in new sessions.
These results encourage us to improve this alternative and include it as a basis in all treatments for children with ADHD, proposing to parents that before this diagnosis start this group and from there each case is raised individually. - Palabras_clave_ingles Attention deficit disorder and hyperactivity groups of psychoeducation treatment ADHT

Resumen_ingles
Introduction: The impact on society of the pharmacotherapy associated with attention deficit hyperactivity disorder (ADHD), invites one to reflect upon the efficiency of current therapeutical strategies. A pharmacoenocomic evaluation is hereby presented in order to quantify the savings associated with the partial substitution of extended release methylphenidate (ER-MPH) for Immediate release methylphenidate (IR-MPH).Methods: A cost-minimization analysis was performed, from the perspective of the Spanish National Health System (SNS by its acronym in Spanish), in a time-span of 3 years. The analysis included IR-MPH and the different ER-MPH options available in Spain; assuming equivalent effectiveness among them. The results are presented as the medication’s direct cost for the SNS. These results were subsequently extended with a budgetary impact analysis (BIA) over 7 years –using an approximated mean of ER-MPH cost. Results: Each patient treated with ER-MPH has an estimated average annual cost of € 343.90, compared with € 91.80 with IR-MPH. Considering a low substitution rate of ER-MPH for IR-MPH (5%), the accumulated savings for the SNS in a period of 3 years would be 4.1 M €. Following a substitution policy of +5% a year, until reaching +25%, would generate an accumulated savings of € 23.7 M; exceeding the SNS’ current yearly investment in MPH (€ 20,9M).Conclusions: Given its low acquisition cost, the increase in the use of IR-MPH would suppose important savings for the SNS. However, other factors should also be considered when implementing this proposal.

Resumen_ingles
Introduction: Attention deficit/hyperactivity disorder (ADHD) affects a significant proportion of the child and adolescent population.Aim: This study focuses on determining the response to immediate- release methylphenidate in children and adolescents diagnosed with ADHD, as well as obtaining information on current treatment patterns and on treatment safety.Subjects and methods: This was a multicentre, retrospective study carried out with 561 patients aged 4-16 years and with ADHD, who had started treatment with immediate-release methylphenidate. The score in terms of DSM-IV-TR parameters, subtypes of ADHD, satisfaction with treatment, use of concomitant medication, and adverse reactions were descriptively analyzed. Results: Attention deficit was more frequent among adolescents from 6-16 years (95.45%) while hyperactivity was more frequent among childrenConclusions: The results overall show a good response to immediate-release methylphenidate and a good safety profile for the treatment.

Resumen_ingles
According to several studies, attention deficit hyperactivity disorder (ADHD) has a prevalence of 5 to 10%. However, the number of patients being seen in pediatric primary care clinics is clearly lower. One possible explanation for this circumstance could be the existence of a window period between the ages of 6 and 11 years during which children only receive medical attention for intercurrent processes, and are not examined in depth as in health monitoring programs. It is at these ages, coinciding with primary school, when the onset of ADHD occurs, with negative consequences on both the academic and the social level.
In order to identify it early, we propose the inclusion of a screening test, specifically Goodman's Strengths and Difficulties Questionnaire, which is available on the Internet and is easy to administer and to evaluate, and refer the possible cases of ADHD to the child psychiatry department where a therapeutic plan will be implemented in coordination with the schools.
