    Introduction: The impact on society of the pharmacotherapy associated with attention deficit hyperactivity disorder (ADHD), invites one to reflect upon the efficiency of current therapeutical strategies. A pharmacoenocomic evaluation is hereby presented in order to quantify the savings associated with the partial substitution of extended release methylphenidate (ER-MPH) for Immediate release methylphenidate (IR-MPH).
    Methods: A cost-minimization analysis was performed, from the perspective of the Spanish National Health System (SNS by its acronym in Spanish), in a time-span of 3 years. The analysis included IR-MPH and the different ER-MPH options available in Spain; assuming equivalent effectiveness among them. The results are presented as the medication’s direct cost for the SNS. These results were subsequently extended with a budgetary impact analysis (BIA) over 7 years –using an approximated mean of ER-MPH cost.
    Results: Each patient treated with ER-MPH has an estimated average annual cost of € 343.90, compared with € 91.80 with IR-MPH. Considering a low substitution rate of ER-MPH for IR-MPH (5%), the accumulated savings for the SNS in a period of 3 years would be 4.1 M €. Following a substitution policy of +5% a year, until reaching +25%, would generate an accumulated savings of € 23.7 M; exceeding the SNS’ current yearly investment in MPH (€ 20,9M).
    Conclusions: Given its low acquisition cost, the increase in the use of IR-MPH would suppose important savings for the SNS. However, other factors should also be considered when implementing this proposal.
